In order to assist you with your homework assignment, I searched the internet to find some sample balanced scorecards. You will find there are many different formats, select the format that best fits your style. Make sure you come up with your own 8 performance measurements and do not copy from the samples.

Remember you also have to justify why you selected the measurements in your scorecard. That can be a paragraph for each of the quadrants.
