Mrs. J. is a 63-year-old woman who has a history of hypertension, chronic heart failure, and sleep apnea. She has been smoking two packs of cigarettes a day for 40 years and has refused to quit.

Three days ago, she had an onset of flu with fever, pharyngitis, and malaise. She has not taken her antihypertensive medications or her medications to control her heart failure for 4 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure.
Subjective Data
Is very anxious and asks whether she is going to die.
Denies pain but says she feels like she cannot get enough air.
Says her heart feels like it is “running away.”
Reports that she is so exhausted she cannot eat or drink by herself.
Objective Data
Height 175 cm; Weight 95.5 kg
Vital signs: T 37.6 C, HR 118 and irregular, RR 34, BP 90/58
Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint; all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation
Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%
Gastrointestinal: BS present: hepatomegaly 4 cm below costal margin
Critical Thinking Questions
What nursing interventions are appropriate for Mrs. J. at the time of her admission? Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?
IV furosemide (Lasix)
Enalapril (Vasotec)
Metoprolol (Lopressor)
IV morphine sulphate (Morphine)
Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition.
Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent problems caused by multiple drug interactions in older patients. Provide rationale for each of the interventions you recommend.

Explain the rationale behind Galton’s ideas regarding heredity.
For the last few weeks, you have followed the many paths leading to modern psychology. The challenge so far has been the diverse viewpoints of the mind and behavior. Two important influences emerged in the late 19th century and they are the emergence of understanding brain function and the influence of hereditary. Francis Galton described human intelligence as being associated with social stature. For instance, it was thought someone with a family background of royalty, science, and medicine would breed more persons of genius than an individual of lower social status. Galton’s ideas were in part developed due to his cousin’s work on understanding evolution. That cousin was none other than Charles Darwin. Psychology as a science was just beginning at this time with little unification on not only understanding behavior but how to even go about studying it.

Explain the rationale behind Galton’s ideas regarding heredity. From current understandings of hereditary traits, genetics and human behavior explain the major issues regarding Galton’s work.
Why did the ideas of Darwin and others like him have a prominent influence on psychology at the time? Explain the strengths and weaknesses of evolution and natural selection with regard to social theory and psychology.
What were the differences and similarities between Wundt and Titchener’s structuralism and Act psychology? Explain why the psychology of Wundt, and especially Titchener, did not last.

Influence of Economics

Purpose of Assignment

To locate, retrieve, and evaluate the effects of macroeconomic indicators on your own decision making.

Assignment Steps

Resources: Tutorial help on Excel® and Word functions can be found on the Microsoft® Office website. There are also additional tutorials via the web offering support for Office products.

Scenario: Consider your last big purchase such as a car, appliances, home repairs, home purchase, computer equipment, college tuition, or another “big-ticket” item, which are often purchased using loans/financing (by borrowing money). Also consider your decision-making process that led you to choose a particular make, model, or brand of the product (or service) you purchased and whether it was the right time to make the purchase given economic conditions at the time of your purchase. While analyzing your decision, keep in mind everything from interest rates to the prices of complementary and substitute goods are driven by human economic behavior.

Develop a minimum 1,050-word analysis of your decision-making process in which you include the following:

  • Retrieve statistics on Real Gross Domestic Product (GDP) and on Real Personal Consumption Expenditures (PCE) by year for the last ten years. You can retrieve those statistics from internet sources including, but not limited to, the Federal Reserve of St. Louis’s FRED web site, the U.S. Department of Commerce’s Bureau of Economic Analysis (BEA) web site, or another credible source of your choice. Post these statistics in a single worksheet of an Excel® workbook and submit your Excel® file with your report. In your report, discuss the latest 10-year trends in both GDP and PCE. Also discuss how the trends in GDP compare with trends in PCE. You are encouraged to include graphs of these statistics in your report; you could create the graphs in Excel®and copy them into your report.
  • Retrieve statistics on the Effective Federal Funds Rate and on the Consumer Price Index: All Items Less Food and Energy by year for the last 30 years. You can retrieve those statistics from internet sources including, but not limited to, the Federal Reserve of St. Louis’s FRED web site, the U.S. Department of Labor’s Bureau of Labor Statistics (BLS) web site, or other credible sources of your choice. Post these statistics in a single worksheet of an Excel® workbook and submit your Excel® file with your report. In your report, discuss how the trends in the Effective Federal Funds Rate compare with trends in inflation. If you took out a loan to pay for your “big-ticket” purchase, what was the interest rate on your loan? Were interest rates rising or falling at that time? Were interest rates relatively high or low at that time? You are also encouraged to include graphs of these statistics in your report.
  • Discuss the influence of any Federal government or state government programs, such as tax credits or tax deductions for energy-saving/efficiency purchases, on your decision to make your last big purchase; or if government incentives did not factor into your decision, explain why not.
  • Develop conclusions about the economy’s influence on personal and business decision-making relative to purchases of big-ticket items, investments, or other major purchases.

Cite a minimum of three peer-reviewed sources. Note: The Federal Reserve of St. Louis, the Bureau of Economic Analysis, and the Bureau of Labor Statistics can be cited to fulfill this requirement.

Format your paper consistent with APA guidelines.

Click the Assignment Files tab to submit your assignment. Be sure you submit your paper as well as the two required Excel® files.

Discussion 1: Impact of Mainstream Focus on Psychopathology and Diagnosis

Diagnosis and the description of mental health issues have recently become ubiquitous in the media. One of the reasons for this new emergence of the topic of mental health in the mainstream media is because of worldwide traumatic events. Many tragic events covered in the media are centered on one or two individuals who have committed acts of violence. Unfortunately, most of these individuals have had a history of mental illness. This aspect of the tragedy then becomes the focus of analysis within the media. From this point of reference, reporters and journalists educate the public on mental health issues. However, these tragedies could be an opportunity to educate the public about the facts related to mental illness. Unfortunately, the media discussions about mental illness often result in attaching a stigma to mental illness and to the population suffering from mental illness.

As a clinical social worker, you need to be aware of the impact of the entry of psychopathology and diagnosis in mainstream public discourses. You also need to think about how the media could help eliminate or mitigate the stigma attached to mental illness in the mind of the general population.

Examine an adverse event at the unit level in your organization or one with which you are familiar and apply the Regulatory Decision Pathway.

Examine an adverse event at the unit level in your organization or one with which you are familiar and apply the Regulatory Decision Pathway.

Review the information on just culture presented in the Learning Resources.
For this discussion, you will use the Regulatory Decision Pathway found in Russell, K. A. & Radtke, B. K. (2014).
Examine an adverse event at the unit level in your organization or one with which you are familiar and apply the Regulatory Decision Pathway.
Compare the findings of the Regulatory Decision Pathway to what actually happened at the unit in your organization. Was the event deemed: bad intent, reckless, at risk, or human error? According to the pathway, do you now think it was the correct action?
Think about how a nurse leader–manager may use just culture as a framework to create or maintain a focus on accountability and outcomes throughout a group. What actions could be taken if a systems–related error was made or if an error resulted from risky behavior?
How might role conflict and/or ambiguity have contributed to the situation?
By Day 3

Post a description of an adverse event in your organization and your analysis of the issue using the Regulatory Decision Pathway. Explain how role conflict or ambiguity might have influenced this situation. Apply the principles of just culture as you explain how you, as the group’s manager, would handle the situation.

Performance Evaluation Assignment

This assignment marks the final component of the term project. You must now design a performance evaluation document for the job for which you previously wrote a job description, recruitment and selection plan, interview questions and an offer letter. Be sure to not only create the evaluation questions/scales, but the form itself.

In other words, what form would you give to a manager and/or the employee if you were the HR director initiating the performance review process? In addition, you should consider creating a form that will provide the manager developmental information and opportunities.
